Nurse Case Management Services
As Needed Sessions
30-, 60-, or 90-minute individualized sessions to assess and meet you or your loved one’s needs within a few visits. Best for current and new clients, wound care, NYS PRI and Screen, and more. This is perfect for clients with simple requirements that may be addressed in a few visits.
Monthly Support
A monthly plan to work on you and your loved one's mild to moderate home care and community resource needs. This includes one monthly session and contact with me between visits via email, phone, and text during regular business hours. This is suitable for monthly medication set fills, health care navigation and support, advocacy, care management education, and more.
8-Week Plan
An 8-week plan to work on you and your loved one's intensive home care and community resource needs. Includes weekly or biweekly sessions (per client needs) and availability to me between visits via email, phone, and text during regular business hours. Best for necessities such as medication education and reconciliation, wound care, advocacy, and addressing multiple complex needs with specific goals.
